A chaotic record, in one clean timeline

The clinician · What’s advancing · F3

The timeline, the contradictions and the gaps — the three things that take an hour to find by hand.

Why this matters for your role

The clinician’s skill: ask which system, population, comparator and outcome were actually studied.

Your worked example

A fictional teaching exercise compares a task-specific imaging system evaluated in a screening programme with a general-purpose chatbot that has not been evaluated in that programme.

Put it into practice

Build a checklist covering intended use, population, comparator, outcomes and validation. Explain why evidence for the imaging system cannot establish the chatbot’s clinical performance. Do not diagnose or recommend treatment.

Practise with a synthetic example. Do not upload identifiable patient information. Check every factual claim and source before considering any real-world use.

Turn this fictional record into a usable chronology. This is a practice exercise; the material below is synthetic. [PASTE A SYNTHETIC OR FULLY DE-IDENTIFIED RECORD — notes out of order, abbreviations, repeated entries] Produce, in this order: 1. A dated timeline of events, one line each, with the source line for every entry. 2. A medicines table: drug, dose, started, stopped, and by whom — with "not recorded" wherever the record does not say. 3. A contradictions list: every place where two entries cannot both be true, quoting both. 4. An omissions list: what a reader would expect to find in a record like this and cannot. 5. The five questions this record raises that only a human can answer. Never resolve a contradiction by choosing the more likely version. Show both. End with a short section titled "Your check" listing what I must verify myself before using any of this.

Synthetic practice example. Run it as-is and check the output against the supplied facts.

Your check

Spot-check five timeline entries against their source lines. The contradictions list is the valuable part precisely because it is where a summariser normally smooths things over.
